When blood sugar remains elevated over time, tiny blood vessels in the retina can become damaged. These vessels may swell, leak fluid, or close off, affecting how the retina captures and sends visual information to the brain. As the disease progresses, the eye may begin to form new blood vessels in an attempt to compensate. However, these vessels are often fragile and can leak, which may lead to additional vision problems.

Stages of diabetic retinopathy

Non-proliferative diabetic retinopathy (NPDR)
Small blood vessels weaken, swell, and/or leak fluid or blood. Vision changes can be subtle or absent.

Proliferative diabetic retinopathy (PDR)
New abnormal vessels grow. Because they’re fragile, they can bleed, leak, or cause scar tissue and pose a higher risk for vision loss.

Possible complications

Macular edema
Swelling in the macula can blur or distort central vision.

Retinal detachment
Scar tissue or abnormal vessel growth can pull on the retina and detach it, which is serious and needs prompt care.

Fluctuating vision
Blood sugar changes can cause temporary shifts in prescription and clarity.
