Whether you’re visiting an eye doctor for the first time or it’s been a few years since your last appointment, knowing what to expect can make the experience feel more comfortable. A comprehensive eye exam goes well beyond checking whether you need glasses. It’s a thorough evaluation of your eye health, your visual system, and how your eyes work together.
At Clear & Co., we take time with each patient. Most comprehensive exams last about 45 minutes to an hour, depending on your history and what we find along the way. Here’s a walkthrough of what to expect.
1. Your health and vision history
Before any testing begins, we’ll ask about your general health, any medications you take, your family eye history, and any vision concerns you’ve noticed. This context shapes the exam. Conditions like diabetes, high blood pressure, and autoimmune disorders can all affect your eyes, so we want the full picture before we start.
If you wear glasses or contacts, bring them along so we can evaluate your current prescription. And don’t hesitate to mention anything, even symptoms that seem minor, like occasional dryness or headaches after screen time.
2. Visual acuity and refraction
This is the part most people think of when they picture an eye exam. We’ll check how clearly you see at various distances using a standard letter chart. Then, using a phoropter (the instrument with all the lenses), we’ll fine-tune your prescription one lens at a time.
There’s no single “right” answer during refraction. We’ll guide you through each comparison and combine the results to find the clearest, most comfortable prescription for your eyes.
3. Eye pressure and glaucoma screening
Elevated eye pressure is one of the key risk factors for glaucoma, a condition that can cause permanent vision loss without any early warning signs. We measure your intraocular pressure using a quick, painless test. If your pressure is elevated or if there are other risk factors, we may recommend additional testing.
Early detection is the best way to manage glaucoma effectively. It’s one of the most important reasons to stay on schedule with your eye exams, even if your vision seems fine.
4. Eye health evaluation
Using specialized instruments and a slit lamp microscope, we examine the structures of your eye in detail, including the cornea, iris, lens, and the retina at the back of your eye. We’re looking for signs of cataracts, macular degeneration, diabetic retinopathy, and other conditions.
Depending on your age and health history, we may dilate your pupils to get a wider view of the retina. Dilation drops take about 20 minutes to take effect and your near vision will be blurry for a few hours afterward, so it’s helpful to bring sunglasses along.
5. Discussion and next steps
After testing is complete, your doctor will walk you through everything we found. We’ll explain your prescription, discuss any areas of concern, and answer your questions in plain language. If follow-up care is needed, we’ll outline the plan together.

How often should you have an exam?
For most adults, we recommend a comprehensive eye exam every one to two years. If you have diabetes, a family history of eye disease, or wear contacts, annual exams are especially important. Children should have their first exam by age one, again before starting school, and annually after that.
